The federal government is sending a one-time $90 payment to about 20.8 million people on traditional Medicare. The money comes from a $2 billion fund called the Medicare Improvement Fund.

Why Does It Look Like It Comes From Social Security?
Because the Social Security Administration (SSA) handles the payment. SSA is the agency that sends out Social Security benefits and manages Medicare premium billing. So the $90 arrives through SSA — but it is a Medicare rebate, not a Social Security benefit increase.
Who Gets It — and Who Does Not?
Only about one in three people with Medicare Part B get this payment.
You may qualify if you are on traditional (original) Medicare with Part B and you pay the standard Part B premium yourself.
You do not qualify if Medicaid pays your Part B premium for you, or if you pay extra because of higher income (these extra charges are called IRMAA surcharges).

Is This a Permanent Increase? Is It the COLA?
No. This is a one-time payment. It will not repeat next month. It is also not the 2026 cost-of-living adjustment (COLA). The 2026 Social Security COLA is 2.8%, and that is a separate increase. Do not confuse the two — what the $90 payment is not clears up the difference.
