A $90 payment showed up in your bank account. It says it is from Social Security. You did not apply for anything. Is it real? Yes — for millions of Medicare beneficiaries, it is. Here is what it is, in plain English.
What Exactly Is the $90 Payment?
The federal government is sending a one-time $90 payment to about 20.8 million people on traditional Medicare. The money comes from a $2 billion pot called the Medicare Improvement Fund.
The purpose is simple: to offset the October 2026 Medicare Part B premium. The standard Part B premium is $202.90 a month in 2026, up from $185 in 2025. The $90 helps cover part of that bill.
Why Does It Come Through Social Security?
This confuses a lot of people. The payment is about Medicare, but it arrives through the Social Security Administration (SSA). That is because SSA handles the money side — it sends out Social Security benefits and manages Medicare premium billing. So when you see the deposit, it looks like it comes from Social Security. It is really a Medicare rebate.
Do I Need to Apply?
No. There is no application. If you qualify, the payment comes automatically — by direct deposit if that is how you get your benefits, or by paper check otherwise. Anyone who tells you that you must "sign up" or "claim" your payment is trying to scam you.
When Did It Start?
Direct deposits started on October 8, 2026. Paper checks go out later in October. The payment is labeled along the lines of "Medicare Improvement Fund Payment; $90 Payment to Offset October Premium." For the full schedule, see when your $90 payment will arrive.
Who Does Not Get It?
Not everyone with Medicare gets this payment — only about one in three Part B enrollees. People whose premiums are paid by Medicaid and people who pay extra because of higher income (IRMAA surcharges) are excluded. The full list is in who qualifies for the $90 rebate.
Is It a Permanent Increase?
No. It is one-time only. Your regular Social Security check does not change. It is also not the 2026 COLA — the 2026 Social Security COLA is 2.8%, and that is separate.
